The Surgery will be closed on Thursday 30th July between 1pm-3pm for Staff Training.

Please note we are encouraging all patients attending appointments or visiting  BHGP to  wear a face covering.



We can all help control the virus if we all stay alert. This means you must:

Stay at home as much as possible

Work from home if you can

Limit contact with other people

Keep your distance if you go out (2 metres apart where possible)

Wash your hands regularly

Self-isolate if you or anyone in your household has symptoms.

Do not go to a GP surgery, pharmacy or hospital.

You do not need to contact 111 to tell them you're staying at home.

Testing for coronavirus is not needed if you're staying at home.

If your child is unwell with a new rash please call the surgery to speak to the duty doctor for advice before coming in.

For Patients who are requiring dressings, including suture removals, please contact the surgery and book an appointment with the  practice nursing team, or the  Dressing Hub. Only attend the A & E's for emergencies.

Virtual Patient Group

Our virtual patient group is now live. If you want more information about it and to register to join please visit the Patient Group tab on the right hand side- select the virtual patient group tab, complete the form and return to us.

Patient Participation Group

If you would like to join the patient group meetings which take place in the surgery please leave your name with reception. The next meeting will be held 28th May at 1 pm, new members welcome.


Accessible Information Standard

The Accessible Information Standard aims to ensure that patients (or their carers) who have a disability or sensory loss receive information they can access and understand. For example in large print, braille or via email.

As part of the Accessible Information Standard we must:

  • Identify patients who may have an additional communication need.
  • Record the patient's need in our clinical GP system.
  • Flag the patient's record to assure additional communication needs are obvious to practice staff;
  • Share this information with other care givers such as social care;
  • Act to ensure our staff can effectively communicate with patients who may have additional communication needs.

Please inform a member of staff in reception if you have any communication support needs- you can call us on 0207 249 5511 or tell a receptionist in person. You need to help us make sure we get things right for you.

Named Accountable GP for all Patients

From 1st April 2015, all practices are required to inform their patients including children of their named accountable GP, under the new GP contract for 2015/2016. The named GP will have overall responsibility for the care and support that the surgery provides to you.

If you would like to know who your named GP is please contact reception.

This does not prevent you from seeing any GP in the practice as you currently do.

Repeat Prescription

Please allow 48 hours for processing of your repeat prescription once you have dropped it off at the surgery. We do not take requests for repeat prescriptions over the telephone.

Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors or nurses, please telephone to cancel your appointment.

Out of Hours

Please ring 111 if you have any Out of Hours emergencies. Telephone calls to the out of hours service are routinely tape-recorded as a safe-guard for patients and staff alike

Registration with the practice during Covid-19


During this difficult time, if you wish to register with Barton House, and your address is within our catchment area, please follow the guidelines below.

Please complete the following forms and submit via email along with 1 proof of ID from Section A, and 1 proof of address, dated within the last 3 calender months from Section B.  Because we provide Online Services, you will also be required to complete an online access form. As part of the registration process, we need some information about you, please complete the Medical Questionnaire, this is because it can take a considerable time for us to receive your medical records and the SCR Consent Form


Section A: Acceptable Photo ID

  • UK passport
  • International Passports
  • Biometric Residence Permit
  • UK/EEA/EU photo driving licence
  • EU/EEA National Identity Card
  • Student ID cards
  • 16 Plus Zip/18 plus Student Oyster Cards
  • 60 Plus Oyster Cards or Freedom Pass
  • Company photo ID cards
  • Armed forces ID cards,
  • Jobcentre Plus Travel Discount Card for the following:- Jobseekers Allowance, Incapacity Benefit, Employment and Support Allowance or Income Support


Section B: We will accept the following as proof of address, dated within the last 3 months:

  • Mortgage Statement – last quarter
  • Recent (last 3 Months) Utility Bill – Gas, Electricity, Water, Land Line Telephone (Not mobile phones)
  • Current Year Council Tax Bill
  • Current year Television License
  • Sky/Virgin/BT/ Broadband/TV Package Statement (last 3 Month)
  • Current Year House or motor insurance certificate
  • Current State Pension notification letter
  • Current/Recent Benefits Agency letter
  • Bank / Building Society / Statement (last 3 months)
  • Solicitor letter confirming completion of house purchase
  • Credit Card Statements from main provider
  • Tenancy Deposit Protection (TDP) schemes Document
  • Current HMRC Tax notification documentation/Other Documentation from HMRC 
  • Letter from Council Confirming Electoral Roll Listing
  • Benefits or pensions notification letter confirming the right to benefit
  • Student Finance Letter
  • Wage Slip if has Address –last 3 months
  • Sponsorship Letter confirming address if from overseas

Please either print the following 4 forms and return by post with the required information (ID/Proof of Address) or send all 3 forms, plus ID/Proof of Address via email:

  1. Covid-19 Signature Letter for Patients
  2. GMS1 - Family Doctor Services Registration Form

  3. Patient Medical Health Questionnaire with Summary Care Record Consent form
  4. Child Medical Health Quetionnaire with SCR Consent

  5.  Online Access to my Medical Records

Accepting and Removing Patients from Our List

We will normally accept any patient living within our practice area onto our list. If you move out of our area, we will ask you to re-register with a more local doctor. In exceptional circumstances, we reserve the right to remove any patient from our list.
